The DOT physical is the medical evaluation the Federal Motor Carrier Safety Administration (FMCSA) requires for commercial drivers; without a current medical certificate, you can't get or keep your CDL. Do You Need One?
As a general rule, you need it if you drive a commercial vehicle in interstate commerce that:
- Weighs more than 10,000 pounds (gross vehicle weight), or
- Carries 16 or more people, including the driver, or
- Hauls hazardous materials that require placards (HAZMAT).
If you only drive within Texas, check with your employer or the Texas Department of Public Safety about which rules apply to you. FMCSA's official guidance is at fmcsa.dot.gov/medical.
Before You Leave Home: Your Checklist
Showing up prepared saves you a second trip. Bring:
- Government-issued photo ID
- Glasses or contacts, if you wear them to drive
- Hearing aid, if you use one
- A list of all your medications, with names and doses
- If you have diabetes, high blood pressure, a heart condition, or sleep apnea: recent notes or results from whoever treats that condition
- If you use CPAP: your machine's usage report
One practical tip: don't leave your renewal for the last day. If something needs extra paperwork, you'll have time to get it without being stuck off the road.
What Gets Checked at the Visit
The evaluation follows the federal exam form (MCSA-5875). In plain terms:

| Physical exam | A look at the heart, lungs, belly, back, arms and legs, and reflexes |
Keep in mind that the urine test in a DOT physical is not a drug test. If your employer asks for one, that's a separate step; the clinic also offers drug and alcohol testing.
When you meet the standards, you're given your medical certificate (form MCSA-5876). If you hold a CDL, remember to submit it to the Texas Department of Public Safety as they instruct.
If You Have a Health Condition
A chronic condition doesn't automatically disqualify you. What matters is that it's under control and documented.
High Blood Pressure
If your reading is elevated, the certificate may be issued for a shorter period, and if it's very high you may not be certified until it's under control. If you know your pressure runs high, it's worth getting it checked beforehand through chronic conditions care.
Diabetes
Many drivers managing diabetes with diet or pills get certified without trouble when control is good. If you use insulin, FMCSA has a specific process that includes a form completed by the clinician who treats your diabetes; bring it filled out.
Sleep Apnea and Other Conditions
Bring proof that you're following your treatment. In special cases, if a result calls for it, the team will help guide a referral to a specialist.
How Often Do You Renew?
A medical certificate can last up to two years. If a condition needs monitoring, the period may be shorter. Put the expiration date in your phone and plan your renewal ahead of time.

Warning Signs That Can't Wait for an Exam
